    • Who is eligible? All caregivers and their dependents aged 18+ enrolled in Amedysis Highmark  medical plan who have been diagnosed with diabetes are eligible for Vida Health at no additional charge.

    • Where should non-eligible members be referred?- If a member is having eligibility issues such as being unable to validate when they try to sign up/register, they should be routed to the Amedisys Benefits Center 888-528-7066; option for benefits questions
    • If a member is not eligible to participate in the T2 program because they don't have diabetes, or some other clinical issue (which is a requirement for Amedisys), they need to be routed to ShareCare (This is set up in the app as well to notify members when no program is available to them.
