Description:
The Vida Whole Health (Diabetes+) program is an evidence-based, multi-dimensional approach designed to support members with diabetes (type 1 or 2) as well as common cardiometabolic comorbidities, namely hypertension, and hyperlipidemia to improve health through diabetes self-management, attaining metabolic goals, education, and support.
Eligibility:
The Whole Health program is available to members with polychronic conditions including diabetes, hypertension, and hyperlipidemia. The member does not have to have diabetes to participate in this program.
Type of Provider For This Program:
- Registered Dieticians
Exclusion Criteria:
- Under 18 years old
- Eating disorder within the last 5 years
- Anyone who appears not able to interact coherently and safely due to dementia or other mental or psychological impairment may be excluded at the provider's discretion.
- Current pregnancy/ Nursing or pumping breast milk/ Planning to become pregnant
- If asked reason, state: "given the nature of the program and potential complexities with pregnancy our clinical team recommends resilience during pregnancy"
Inclusion Criteria:
- BMI >= 18.5
- Must have one or more chronic conditions: for example, Diabetes, Congestive Heart Failure, Hypertension, Arrhythmia, etc
- Members with Type 1 and 2 Diabetes

Goal(s):
The program goal for DM+ is now considered flexible and coordinated with the coach and member. The PRIMARY goal for this program is to aim for on-target A1C.
- A1c 5.6% without DM or 6.5% with DM or Per MD or individualized goal, if more appropriate
- 5-7% weight loss by 6 months for members with <8% A1C at baseline
- 7 days a week taking medication as prescribed
Internal Metrics:
- Medication Adherence of 80% weekly by 6 mos - Self-reported and Pharmacy data
- Of those who are non-adherent at baseline, at least 25% become adherent during the 6 months following onboarding.
- Engagement % of members that meet the definition of active engagement - 75% at month 3

We’re sorry, you're not eligible to receive a glucometer through Vida.
Since we provide these devices free of charge, we have an eligibility criteria in place to ensure they go to those with higher acuity. For example, for someone who's on a combination of medications like metformin + insulin or GLP-1s, it's determined that consistent blood sugar monitoring is critical for their diabetes management. Therefore we supply them with a blood sugar meter.
In a scenario like yours, someone who's [[only on metformin//not on medications]] to manage their diabetes, it's determined that the best approach to help you improve your A1C would be through behavior changes such as eating healthier and mindful awareness. Something our coaching programs are built to do. This doesn't mean you are without a device in your journey: we do offer a weight scale for someone with your level of acuity, to help focus on weight loss as part of your diabetes management.
