Member-facing name: Back Pain Relief
Program Manual VP Card
Description:
Vida’s MSK program is designed to be a Physical Therapist supervised, Health Coach led program to help members reduce lower back pain, improve function, and prevent future back pain exacerbations. The member will be expected to complete 3 exercise sessions a week and to walk on days they are not exercising for 10-15 minutes. The program is designed to provide a customized exercise pathway for the member with 4 different classifications and 3 levels of exercise for each classification for both English and Spanish languages. Coaches will connect with the member weekly throughout the program to assist the member with accountability, goal setting, education and monitoring of their progress, and will escalate members as necessary to a licensed Doctor of Physical Therapy for additional consultation as necessary.
Members are screened prior to entering the program using a questionnaire that assesses their current functional level, symptoms, limitations, and mindset of their injury. Using the Oswestry Disability Index, and other curated information, we can ensure the member is placed within a pathway of exercises that addresses the complexity of their spinal dysfunction, and safeguards have been instituted to verify and confirm these classifications are ideal for the member’s limitations.
