Description:
The COPD program is designed to support behavioral change in medication adherence and energy conservation for individuals suffering from COPD as well as assist them with identifying and managing triggers that exacerbate their clinical condition.
For individuals suffering from both COPD and Asthma, content for both conditions may be sent to the member.
Coaches offer accountability and support for making healthy changes to reduce member's risk of other complications, enhance their wellbeing, and prolong their health.
Eligibility:
The program is designed to support members with COPD and those who have a combination of COPD and asthma.
Inclusion Criteria:
- Diagnosis of COPD or emphysema (self-reported ok)
- Currently on medications for COPD
- Interested in learning self-management techniques
- Must have Asthma
Exclusion Criteria:
- Currently on palliative or hospice care
- No diagnosis of COPD or emphysema
- Not on medications for COPD
- Under the age of 18
- Pregnant
Type of Provider For This Program:
- Health Coaches
Goal(s):
Program Goal: For members to achieve 80%+ use of their controller inhaler(s) as prescribed; measured via “Controlled Inhaler Puffs” Tracker.
Behavior Goals: Each Behavior goal is set with the coach-provider & member in the corresponding tracker.
- Medication Adherence - “Using Inhalers” - 80% adherence to taking controlled inhaler
- Trigger Management -“Managing Triggers” - Reduction in rescue inhaler uses per day
- Energy-conserving Strategies “Breathing & Energy” - Improvement in breathing exercises daily
- Mindful Awareness “Being Mindful” - Increase in the mindfulness activities practiced
- Smoking Cessation “Quitting Smoking” - Zero cigarettes per day
