Description:
The Asthma program is designed to support behavioral change in medication adherence and physical activity in individuals suffering from Asthma as well as assist them with identifying and managing triggers that exacerbate their clinical condition.
For individuals suffering from both COPD and asthma, content for both conditions may be sent to the member.
Coaches offer accountability and support for medication adherence and making healthy changes to reduce member's risk of other complications, enhance their wellbeing, and prolong their health.
Eligibility:
The program is designed to support members with asthma.
Inclusion Criteria:
- Diagnosis of asthma (self-reported ok)
- Currently on medications for asthma
- Interested in learning self-management techniques
- Must have COPD (Chronic Obstructive Pulmonary Disease)
Exclusion Criteria:
- No diagnosis of asthma
- Not on medications for asthma
- Under the age of 18
- Pregnant
- Is in hospice care
- Does have CHF
Type of Provider For This Program:
- Health Coaches
Goal(s):
Program Goal: For members to achieve 80%+ use of their controller inhaler(s) as prescribed; measured via the “Controlled Inhaler Puffs” Tracker.
Behavior Goal(s): Each goal is set between the coach & member in the corresponding tracker.
- Medication Adherence - “Using Inhalers” - 80% adherence to taking controlled inhaler
- Trigger Management - “Managing Triggers” - Reduction in rescue inhaler user per day
- Exercise - “Being Active” - Improvement in regular physical activity via minutes per week
- Mindful Awareness - “Being Mindful” - Increase in the mindfulness activities practiced
- Smoking Cessation - “Quitting Smoking” - Zero cigarettes per day
