Use Case 2: Vida as an option to receive GLP-1s through MWL [Executed at launch]
- Some members may go through their primary care physician for GLP-1 access, and others will go to Vida for it. For the latter, members have access to obesity treatments via Vida's Medical Weight Loss program, which includes an extensive medical intake with labs. They receive MNT and health coaching. They will be evaluated as candidates for AOM at the start of the program and throughout, based on their weight loss progress.
- NOTICE: Dentex (National Dentex Corporation/CapRx) is ending their members’ access to the Vida Prescribing Program only starting July 1st.

Device Prescribing Details
Vida will write prescriptions for glucometers, test strips, lancets, and Continuous Glucose Monitors for Authorized Users based on the Partner formulary, and will prescribe formulary-aligned medications prioritized based on the tier the member is in.
- We are NOT provisioning these devices - we will prescribe them and send script to the member's pharmacy for them to fill.
- For device issues, members will need to contact the manufacturer
- BP cuffs and scales will be provisioned as normal (not prescribed)
Contour Next coupon
- CapRx offers a coupon to obtain a free Contour Next glucometer.
- Sample Coupon for reference.
- One time use per member. Members can contact their insurance provider to obtain a coupon.
- In the form they fill out prior to us prescribing, it will say: Your Vida medical provider can write a prescription for a blood sugar meter kit. It also has a lancing device, lancets, and 1 container of test strips. Here’s a coupon to help pay for the meter.

- Out of the ~2k members, 30 of them are on Livongo but NDX has been unable to obtain from Livongo the list of these 30 members. We are proposing to NDX to send a comms to their employees to contact us if they are on Livongo and are currently on meds/devices, so we can support them appropriately

Included will then reach out to Capital Rx on the dedicated backdoor line 332-345-4162 if needed. When Included Health calls on National Dentex's backdoor line, member validation and consent are NOT required before disclosing PHI/PII.
