How to Share Your Documents for Quick Processing

Prepare Your Documents: PDF is Preferred ✅

We strongly prefer a PDF file as it ensures all necessary information is clearly visible, including your name, date of birth, the lab name, collection date, and the full results.

• Use a PDF (Preferred): If you already have a PDF file, download that file and follow the instructions below on how to share with Vida.
• No PDF? Scan the document to make a PDF file.

How to Download Labs from MyChart/Portals (Saving as PDF)

1. Log in to your MyChart or lab portal.
2. Navigate to Test Results.
3. Click on a specific test, such as "Lipid Panel," one by one.
4. Look for either a printer icon or a three-dot menu ($\cdots$) on the top of the screen; click on Print.
5. In the print options, look for the share icon ($\boldsymbol{\uparrow}$ with a box on the bottom of it).
6. Scroll down to "Save to Files."
7. If on a phone: The file will save to your phone, iCloud, or Documents. Use the left arrow to browse and select the correct file location.
8. Tip: There should be a way to rename the file before saving it.
9. Use your file folder app to confirm the saved file.

 Alternative Methods (Photos/Screenshots) 📸

If a PDF is not possible, you may take a picture or screenshot.

• Paper results (Take a picture): Lay your paper flat on a surface with good lighting. Open your phone camera, hold it over the page, and move your phone until the results fit in the screen. Take a few pictures in a row in case one is blurry.
• Online results (Screenshot/Print to PDF):
  • Open the lab results in the medical office or lab online portal. Many have an option to "Print" your results. You can use this print function to save an electronic PDF to your phone or computer (PREFERRED).
  • Or you can take a "screen grab" or "screen shot." It’s OK if all the results don’t fit in 1 picture. Make as many pictures as needed to show all the results.

• Your full name
• Your date of birth
• Date of test
• Test name
• Test result or “value”

Medical Records: 

If you’ve already started a GLP-1 medication, you'll need to provide either your medical records, a document on your physician’s letterhead, or an after-visit summary that includes the following information in order to get started with our program:

• Your name and DOB
• Name and dose of your weight loss medicine
• Date you started (got the first prescription)
• Weight or BMI on that date (note: if they currently have a BMI >30 patients can see the VMP without their BMI at time of initial prescription)
• Name of physician that prescribed medication

Labs:

Labs are tests such as Lipid panel, Comprehensive metabolic panel (CMP), Thyroid stimulating hormone (TSH), and A1C. Now it’s time to share them with Vida so you and your provider can make a plan.

First gather any lab reports from the past 12 months. You may have paper copies. Or an email with a PDF report attached. Or access to see results on your lab or medical office online portal.

Lab result documents must include the following information:

• Your name
• Your date of birth
• Date of test
• Test name
• Test result or “value”

Sharing your documents:

• Upload your image or file to Vida. You can upload your documents to the Documents section of the Settings screen.
  
• Tap "Documents"
   

• Tap "Upload New Document"
  

• Select "Single PDF" to upload a PDF file or "Screenshots" to upload a screenshot from your device
  
   

• Tap the down facing arrow on the "What type of document is this?" box.
  
   

• Tap the applicable document type (Lab result)
  
   

• Tap inside the "Add files" box
  
  
   

• Choose the location of the labs or tap "Take Photo" to take a new photo of them
  

• Tap to select the images/documents you wish to add
  

• Tap "Add" in the top right corner
  
   
• Tap "Save"
  

• You'll see the progress bar go from "Uploading record" to "Upload complete"
  
• Tap "Done"
  

• You'll then see all the documents that have been uploaded. Tap "Upload New Document" if you'd like to upload another file or image.
