Understanding Your Eligibility for Vida Health Devices

As part of your Vida program, you may be eligible to receive a connected health device (such as a Blood Pressure Monitor, Scale, or Blood Glucose Meter) to help you and your provider track your progress.

Important Note: Your eligibility for any device is subject to both Vida’s clinical criteria and the specific health offerings provided by your organization (employer or health plan). All device criteria are subject to change based on clinical best practices and program updates.

General Requirements for All Devices

To be considered for any device, you must meet the following baseline criteria:

You must have completed at least one full provider consultation within the Vida program in the last 30 days.
You must not already own the device you are requesting (you are ineligible if you currently own it).
Members will be limited to a maximum of two (2) devices based on clinically determined prioritization criteria.

Device-Specific Eligibility Criteria

Eligibility for a specific device is based on your medical conditions and the Vida program you are enrolled in.

1. Blood Pressure Monitor (BP Cuff)

Your Conditions & Program	Eligibility Status
Hypertension (HTN) Diagnosis (from claims or self-report) AND Enrolled in the Whole Health (WH) pathway AND DO NOT have moderate to severe diabetes or CHF.	Eligible to receive a BP Cuff.
HTN Diagnosis AND Moderate/Severe Diabetes or CHF AND Enrolled in WH or Heart Failure Focus pathway.	Eligible ONLY if you meet specific clinical criteria (Elevated BP, measure of 140/90 or higher).
HTN Diagnosis AND Enrolled in the Medical Weight Loss or Condition Control pathway.	Eligible ONLY if you meet specific clinical criteria ( Elevated BP, measure of 140/90 or higher).

2. Blood Glucose Meter

Your Conditions & Program	Eligibility Status
Diabetes Diagnosis (from claims or self-report) AND Enrolled in the Whole Health (WH) pathway AND You are on specific antihyperglycemic medications (sulfonylurea, insulin in any form, or oral/injectable hypoglycemic agents excluding Metformin).	Eligible to receive a Blood Glucose Meter.

3. Continuous Glucose Monitor (CGM)

This table shows the conditions you must meet to receive a CGM and explains the one way your order can be placed.

Condition for Eligibility	Requirement Details	Important: How to Place Your Order
Diabetes Diagnosis	You have a confirmed diabetes diagnosis (either through claims or reported to your Care Team).	All CGM orders must be placed through your app. Our Support Team cannot manually place orders.
Program Enrollment	You are currently enrolled and active in the Whole Health (WH) pathway.	The electronic order form will appear in your app only after you meet all eligibility requirements.
Insulin Use	You are currently using Insulin (in any form).	Even if your external provider recommends a CGM, we are unable to fulfill the request if you do not receive and complete the official order form in your app.
A1c Requirement	You submitted an A1c result from the last 6 months that is 8.0 or higher.	This ensures your insurance and clinical team can approve your device.
If All Conditions Met	Your Care Team will confirm your eligibility to receive a CGM.	The necessary order form will appear in your app shortly after confirmation.

4. Scale

Your Conditions & Program	Eligibility Status
Enrolled in Whole Health (WH) pathway AND focus on CHF AND you DO NOT have diabetes.	Eligible to receive a Scale.
Enrolled in WH pathway AND have Diabetes AND you DO NOT have HTN AND are NOT on specific antihyperglycemic medications (sulfonylurea, basal-bolus via multiple daily injections, patch-pump, or pump, or oral/hypoglycemic agents excluding Metformin).	Eligible to receive a Scale.
Enrolled in WH pathway AND have Prediabetes or Obesity AND NO other chronic condition (diabetes, CHF, HTN).	Eligible to receive a Scale.
Enrolled in Condition Control AND have CHF or COPD as a condition.	Eligible to receive a Scale.
Enrolled in Heart Failure Focus, DPP, Medical Weight Loss, or Weight Loss+ pathway AND have Obesity.	Eligible to receive a Scale.

5. Oximeter

Your Conditions & Program	Eligibility Status
Enrolled in the Condition Control pathway AND have COPD as a condition.	Eligible to receive an Oximeter.
