Description:
Based on the evidence-based program Mindfulness Based Stress Reduction developed by Dr. Jon Kabat Zinn.
This program is largely based on the stress-relieving benefits of practicing mindfulness. This is done through meditation, yoga, guided imagery as well as informal practices, mindful eating, mindful communication, etc.
Designed to support members interested in managing stress through gaining skills and learning research-based mindfulness methods to cope with stress.
Type of Provider For This Program:
- Health Coaches
Eligibility:
The program is designed to support members that identify stress management as their main wellness goal.
Inclusion Criteria:
- Member identifies stress management as their main goal
- Consent form completion (electronically signed) in the Programs tab.
- Completion of initial consult to ensure the member is adequate fit for MBSM
Exclusion Criteria:
- Significant health problems where breathing exercises or yoga are problematic. Have the client get PCP approval first and use discretion when sending exercises.
- Moderate-severe depressive symptoms. A high “Stress Score” may indicate a need for a higher level of emotional support.
- Under the age of 18
- Guest Coaching is permitted in the MBSM program EXCEPT for members diagnosed with Type 1 diabetes requesting a guest coach to focus on nutrition or weight loss goals. If a guest coach is added into a member's MBSM program for nutrition please confirm and document in the provider notes the member does NOT have a diagnosis of Type 1 diabetes.
- PHQ and/or GAD >= 15 on most recent assessment, within 6 months → schedule Care Navigator (CN) assessment
- Member displays symptoms of mental distress₁ → schedule CN assessment
- Concern for immediate threat to self or others → emergency protocol

Goal(s):
Program Goal: Reduce Perceived Stress Scale (PSS) by 50% at program completion. The PSS will be used to measure stress levels and is sent automatically every 5 weeks (see “sequenced auto-sent” table below). This scale is used because stress is a constant - your perception of it is what helps you manage it.
