Informed Consent (Required for All Programs)
To begin your program and successfully schedule your first consultation, you must complete two mandatory steps: accepting the Clinical Services Informed Consent terms and submitting the Emergency Contact Information form. This guide provides step-by-step instructions for members onboarding via the mobile app or phone assistance, ensuring all required agreements are finalized. Completing your To-Do List quickly prevents delays in starting your care journey. The process for accepting the Informed Consent depends on how you first accessed the Vida app. Please contact support@vida.com if you have any questions.
If Onboarding Yourself through the Mobile App
During the initial app setup, you will be prompted to agree to our clinical policies.

You will see a checkbox that says "I have read and agree to the Clinical Services and Practice Policies Agreement".

To review and/or print the terms in full before agreeing, tap the "Clinical Services and Practice Policies Agreement" URL. This will open up your browser.

When finished reviewing, close the browser to return to the onboarding page.

Check the box to agree to our Informed Consent terms.

If Assisted with Onboarding (Over the Phone)
If a Health Guide or Support Agent helped you set up your account over the phone:

After downloading and opening the mobile app for the first time, you will see a pop-up requesting that you accept our Terms of Use and Clinical Services and Practice Policies.

Tap the URLs to review the terms in full.

Once you complete your review, tap Accept to agree to our terms.

Form: Member Contact Information
This form is available to you immediately upon enrollment and completing it is crucial before your consultation can be scheduled.
A. How to Complete the Form

Open the Home screen of the Vida app and tap “See All” under the “To-Do List”.

Tap “Form: Member Contact Information”.

Tap each empty box in the form and type in the required details.

After typing in all the information, dismiss your keyboard by tapping “Done” (or the down arrow on Android), then tap “Complete”.

Note that responses cannot be changed after sending. When ready, tap “Send” on the pop-up to send the form.
B. How to Verify Submission & Internal Agent Guidance
Verification ensures the member is ready for the next step, such as scheduling their first appointment.

Open the Vida app and navigate to your To-Do List. Your “Form: Member Contact Information” will be listed here.

Member Verification: Once the form is successfully submitted, its status will immediately change to "In Progress." You should use this status to confirm your submission, as the green check-mark may take some time to appear.

Informed Consent for Therapy and Care Navigation
Members enrolling in our Therapy Program will have an additional step:

You will see an additional item on your To-Do List on the Home screen to complete your Informed Consent for Therapy. Tap to open the form.

Review the terms by tapping the URLs for Privacy Policy and Terms of Use.

To agree to the terms, type your name in the box provided, then tap Complete.
